Responsibilities:
- Assess home health and hospice patients to identify physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and families on care, medications, diet, and home safety, promoting self-care practices.
- Supervise and provide clinical direction to home health aides, LVNs, and STNAs.
- Perform on-call responsibilities and serve as a liaison among patients, families, and healthcare team.
