We’re currently looking for a Primary function is to administer skilled nursing care to patients in their place of residence, as prescribed by the physician and in accordance with the established plan of care. The LPN/LVN works under the supervision of a Registered Nurse (RN) and coordinates care with the interdisciplinary team as well as the patient/family. Responsibilities:
- Assist in providing nursing services and identifying patients' physical, psycho-social, and environmental needs under RN supervision.
- Participate in planning, coordinating, and implementing care to deliver quality, cost-effective patient care.
- Communicate effectively with patient/family, co-workers, and healthcare professionals, documenting outcomes of interventions.
- Assist patients and families in learning self-care techniques and assess their responses to care.
- Support the RN and/or physician in performing specialized duties, including end-of-life care for hospice patients.
