Job Details:

Job Title: Registered Nurse - Home Care

Location: Bala Cynwyd, PA

Setting: Hospital

Pay Range: 2349.20 - 2466.66

Schedule: 08:00 - 16:00

Contract Length: 13 weeks

Candidate Type: Traveler

Requirements: RN with 2+ years recent and relevant home care experience, OASIS and Epic experience required

Responsibilities:

Conduct home care visits, including start of care and revisits
Manage a caseload of 25-30 visits per week
Assess patient needs and develop individualized care plans
Document visits accurately in OASIS and Epic systems
Coordinate with healthcare teams to ensure quality care
