Job Title: RN HH -Clinical Manager

Location: Clearwater, FL

Setting: Home Health

Pay Range: 2182.04 - 2291.14

Schedule: M-F 8am-5pm

Contract Length: 13 weeks.

Requirements: Must possess a valid state driver's license and automobile liability insurance Must be able to drive an automobile in all types of weather conditions Must have an RN license in the State of employment. Home Health, HCHB EMR, and OASIS experience. Supervisory/Management experience highly preferred. 

Responsibilities:

• Ensure the client's plan of care is individualized, appropriate, executed as written, and reassessed by the appropriate healthcare professional during significant health status changes, at the physician's request, and after hospital discharge.

• Verify and process evaluation packets, OASIS, and 485s for accuracy, follow up on documentation requiring correction, and ensure proper start of care and episode dates in HCHB.

• Lead referral intake and management to ensure assessment visits are scheduled and performed timely; participate in weekly case conferences and address recertification/discharge decisions.

• Review and approve case conference notes, enter coordination notes within 48 hours, and contact physicians for orders for continued services or add-ons; route orders for physician signature and ensure corrections are made before approval.

• Update medication profiles, ensure existing orders for medical supplies, and follow up on order issues from tracking reports.

• Document non-admission details in coordination notes if no visit was made, with prior branch director approval; review vital sign alerts, wound score deviations, and follow up accordingly.

• Perform patient visits and participate in on-call rotation as needed; verify billing, benefits, and authorization for non-Medicare patients.

• Review on-call reports, follow up on billing deficiencies within 24 hours, and manage missed and declined visits, including re-scheduling and reassignment tasks.

• Assess lab reports, forward them to the PSC for physician faxing, and function as staff nurse PRN.

• Maintain knowledge of federal, state, and local laws and guidelines governing home health operations; communicate effectively and demonstrate excellent customer service and computer skills.
