Job Title: CWHH-Physical Therapist

Location: Clearwater, FL

Setting: Home Health

Pay Range: 2565.60 - 2693.88

Schedule: Discuss with recruiter.

Contract Length: 13 weeks.

Candidate Type: Traveler

Requirements: Current State License as a Physical Therapist; Proof of current CPR certification; Current driver's license with reliable transportation and insurance; 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting; Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.

Responsibilities:

• Perform clinical assessments to identify patient needs and determine appropriate therapy plans.

• Develop and review plans of care, progress notes, and clinical documentation, revising as needed based on ongoing assessments.

• Plan and provide therapy treatments for neuromuscular, neurological, and orthopedic disorders following medical referral and assessments.

• Instruct patients and families in transfer techniques, equipment use, and home modifications to ensure safety and effective therapy.

• Educate patients and families on therapeutic procedures considering age, condition, culture, and learning readiness.

• Coordinate with interdisciplinary team members to deliver patient-centered care.

• Apply infection control and universal precautions in all patient care activities.

• Accept clinical assignments aligned with education and competence.

• Perform additional duties as assigned.
