Job Title: OT Home Health Therapy/Rehabilitation
Location: Tampa, FL
Setting: Home Health
Pay Range: 2243.33 - 2355.50
Schedule: 08:00 - 17:00
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: Must have home health experience; BLS certification required; 1 year experience required; Valid driver's license and car to make home visits; Must be COVID vaccinated or have exemption form on file; Flu shot or declination with mandatory mask wearing at all times; Must have a minimum of 6 visits per day, with 6 points minimum, possibly increasing by 1 point at manager’s discretion; Must have laptop, MiFi, portable printers; Must be vaccinated as per client policy.
Responsibilities:
- Provide skilled home health visits in patients’ homes, including med/surg, cardiac, ortho, and neuro cases.
- Conduct 6-8 visits daily, including weekends as scheduled.
- Complete documentation using BayCare forms and Ace Timesheet system.
- Adhere to all safety protocols, including mask mandates and vaccination requirements.
- Self-schedule visits and hours based on the needs of the HomeCare office.
- Utilize portable equipment to efficiently serve patients across assigned locations.
- Maintain compliance with Medicare, Medicare Advantage, and Commercial insurance standards.
- Collaborate with the care team to deliver comprehensive patient care and support recovery.
- Manage documentation and reporting accurately to ensure quality standards are met.
