Setting: Home Health





Pay Range: 2518.40 - 2644.32





Schedule: Discuss with recruiter.





Contract Length: 13 weeks

Requirements: Current State License as a Physical Therapist - Required; Proof of current CPR certification - Required; Current driver's license - Required; Must have reliable transportation and insurance - Required; 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required; Previous home care experience - Preferred





Responsibilities:

• Perform clinical assessments to identify patient needs and appropriateness of care.
  
• Document, review, and complete a Plan of Care (POC), progress notes, and other clinical records; revise as needed based on ongoing assessment data.
  
• Plan and deliver treatment for neuromuscular, neurological, and orthopedic disorders based on referrals and assessments.
  
• Instruct patients and families in transfer techniques and equipment use, ensuring safety and proper technique.
  
• Educate patients and families about therapeutic procedures tailored to their age, condition, and learning readiness.
  
• Supervise and coordinate patient care activities with interdisciplinary team members.
  
• Apply infection control protocols to prevent contamination and disease transmission.
  
• Accept clinical assignments aligned with education and competencies.
