Job Title: CWHH-RN Home Health
Location: Rocky Mount, NC
Setting: Home Health
Pay Range: 1902.83 - 1997.97
Schedule: Discuss with recruiter.
Contract Length: 13 weeks.
Candidate Type: Traveler
Requirements: Proof of current CPR certification; Current state license as a Registered Nurse; Home health experience 2+ years; 1-2 years experience as a Registered Nurse in a clinical care setting; Homecare Homebase Software Experience - Required; OASIS Experience - Required; Reliable transportation and insurance - Required.
Responsibilities:
- Assess home health patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
- Develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and families regarding care, medications, and safety, using appropriate teaching methods.
- Provide supervision and clinical directions to home health aides and licensed vocational nurses.
- Perform on-call responsibilities and deliver on-call services as assigned.
