Job Details:
Job Title: Hospice Registered Nurse
Location: Green Bay, WI
Setting: Home Health
Pay Range: 2125.00 - 2231.25
Schedule: 08:00 - 16:30
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: 1 year of skilled home health experience; BCLS; Pending license accepted (OR & WA LIC REQ); Oasis experience required.
Responsibilities:

  • Provide patient care in a home health setting consistent with hospice standards.
  • Visit up to six patients per day while maintaining a 1:1 patient ratio.
  • Complete Oasis documentation accurately and timely.
  • Coordinate transportation and care planning with the healthcare team.
  • Adhere to facility policies including COVID vaccination and booster requirements.
