Pay Range: 2315.40 - 2431.17
Schedule: 08:00 - 17:00
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: Must have home health experience; OASIS training preferred. Certs required: BLS, state licensed. Must have 1 year experience. Must have valid driver’s license and car to make home visits. Must be COVID vaccinated or with exemption form on file. Must have flu shot or declination and in both cases mandatory mask wearing at all times while at work.

Responsibilities:

• Provide skilled home health visits to patients with various medical conditions.
• Assess patient needs and develop individualized treatment plans.
• Document patient progress and treatment outcomes accurately.
• Educate patients and their families on medications, safety, and injury prevention.
• Use equipment such as gait belts, wheelchairs, and other devices during therapy sessions.
