Job Details:
Job Title: Home Health RN: Case Manager
Location: Vienna, WV
Setting: Home Health
Pay Range: 2243.80 - 2355.99
Schedule: Day shift M-F
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: Associates or Bachelor’s Degree in Nursing or related field. Current, unrestricted state Registered Nurse license. Ability to analyze complex medical information and make rational decisions. Case Management certification.
Responsibilities:
- Perform comprehensive assessment of high risk members.
- Collaborate with primary care providers to develop individualized care plans with specific interventions.
- Address, identify, and continuously reassess appropriate levels of care, including non medical supports.
- Coordinate care transitions and engage specialty resources as needed.
- Keep detailed records of clinical, functional, and fiscal outcomes during management.
