Job Title: PT Home Health Therapy/Rehabilitation

Pay Range: 2354.40 - 2472.12
Schedule: 08:00 - 17:00
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: Must have home health experience; OASIS training preferred. Certs required: BLS, state licensed. Must have valid driver’s license and car to make home visits. Must be COVID vaccinated or with exemption form on file. Must have flu shot or declination and in both cases mandatory mask wearing at all times while at work.

Responsibilities:

• Provide skilled home health therapy services including assessment, documentation, and patient education.
• Manage patients with diverse conditions such as ortho, neuro, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, and others.
• Utilize equipment and devices such as gait belts, wheelchairs, and oxygen.
• Develop and implement home exercise programs and medication management plans.
• Ensure safety and injury prevention while maintaining compliance with documentation and billing requirements.
