Job Details:
Job Title: Case Manager | RN - Registered Nurse
Location: Covington, GA
Setting: Home Health
Pay Range: 2197.40 - 2307.27
Schedule: M-F 8am-5pm; On call 7 days once a month
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: HCHB; OASIS; Home health experience; Associates or Bachelor’s Degree in Nursing or related field; Current, unrestricted state Registered Nurse license; Ability to analyze complex medical information and make rational decisions; Case Management certification
Responsibilities:
- Perform comprehensive assessments of high-risk members and continuously reassess appropriate levels of care.
- Collaborate with primary care providers to implement individualized, comprehensive care plans.
- Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.
- Engage specialty resources and arrange non-medical support to promote treatment plan compliance.
- Maintain detailed records of clinical, functional, and fiscal outcomes while identifying opportunities for health promotion and illness prevention.
