Responsibilities:
- Provide outpatient physical therapy services to patients in accordance with physician referrals
- Conduct patient assessments and develop individualized treatment plans
- Document patient progress and treatment plans in EMR systems
- Collaborate with healthcare team members to ensure comprehensive patient care
- Educate patients on injury prevention and wellness strategies
