Job Details:
- Job Title: Home Health PT
- Location: Colorado Springs, CO
- Setting: Home Health
- Pay Range: $2400-$2600/weekly
- Schedule: 8-Hour Days
- Contract Length: 13 weeks
- Candidate Type: Local and traveler friendly
- Requirements: 1 year of experience as a PT, Valid Physical Therapist license, Be willing to obtain Colorado or compact licensure
Responsibilities:
- Deliver one-on-one, high-touch care to patients in their homes
- Conduct skilled nursing visits and perform assessments
- Create and manage individualized care plans
- Collaborate with families and interdisciplinary providers to improve patient outcomes
