- Schedule: 07:00:00 to 15:00:00
- Contract Length: 13 weeks
- Candidate Type: Local and traveler friendly, Local candidates only – travel stipend not available, Traveler friendly – travel reimbursement available
- Requirements: Years of experience as a PTA, Qualified applicants MUST have at least Years of experience in Home Health, Valid Physical Therapy Assistant license, Be willing to obtain STATE licensure
Responsibilities:
- Deliver one-on-one, high-touch care to patients in their homes
- Conduct skilled nursing visits and perform assessments
- Create and manage individualized care plans
- Collaborate with families and interdisciplinary providers to improve patient outcomes
