Schedule: Weekdays and weekends; 15 patient visits per day on weekdays, 40 on weekends; on-call not required

Highlights:

• Work with a 255-bed facility using Epic electronic medical records
• Manage case load of approximately 15 patients daily with interdisciplinary support from social services and transportation

Requirements:

• Active California RN license (permanent license required; temporary licenses not accepted)
• Minimum 3 years of inpatient case management experience
• Demonstrated knowledge of HMO and Medicare for inpatient, home health, and skilled nursing facility settings
• Epic charting system experience required
• BLS certification current and valid
• Proficiency in care coordination, discharge planning, utilization review criteria (InterQual and Milliman Guidelines), and regulatory compliance including HIPAA, CMS, Joint Commission standards, and ICD-10/CPT coding
• Two supervisor references from within the last year required at time of submission
