Start Date: April 13, 2026
Location: Kaneohe, Hawaii
Compensation: $88/hour; details provided to qualified applicants
Schedule: Full-time with rotating weekends (2–3 per month); on-call shifts approximately 6 times per month as needed, including weekends

Highlights:

• Home health nursing role providing care across central and leeward areas of the island
• Case management focus with complex patient populations including frequent emergency department utilizers and chronic care patients
• Collaborative role serving as liaison between patients, families, physicians, and multidisciplinary care teams
• Experience with OASIS documentation and Home Base EMR required
• Mileage reimbursement provided at $0.43 per mile
• Travel nursing experience valued and required for this assignment

Requirements:

• Active Hawaii RN license (must be obtained prior to start date; allow 15–20 business days for processing)
• Minimum 2 years of experience in a Medicare-certified home health agency
• Previous travel nursing experience required
• Proficiency with Homecare Home Base and OASIS documentation
• Valid driver's license with current auto insurance and reliable personal transportation
• Current COVID-19 and influenza vaccinations (declinations not accepted)
• Bachelor's degree in nursing and case management experience preferred
