Location: Kaneohe, Hawaii
Compensation: Details provided to qualified applicants
Schedule: Rotating weekends as needed (2-3 per month); 6 on-call shifts per month including weekends

Highlights:

• Home health coverage serving the Leeward side including multiple communities across Oahu
• Case management role coordinating care across the continuum from acute to home settings
• Utilizes Homecare Home Base and OASIS documentation systems
• Mileage reimbursement provided at $0.43 per mile
• Focus on complex patients including frequent ED utilizers, chronic pain, and underserved populations
• Collaborative interdisciplinary approach with physicians and care teams

Requirements:

• Active Hawaii RN license (plan to arrive early for in-person licensing procedures)
• 2+ years of nursing experience required
• 6+ months of Home Health experience required
• Previous travel nursing experience required
• Valid driver's license, active auto insurance, and reliable transportation required
• Proficiency with Homecare Home Base and OASIS documentation required
• Current COVID and Flu vaccinations required (declinations not accepted)
• Bachelor's degree in Nursing preferred
• Case management experience preferred
