Schedule: Full-time with every other weekend requirement; Days 1:25 patient ratio, Weekends 1:45 patient ratio; 40-hour orientation; Up to 7 days RTO available; Must commit to working 3 of 5 major holidays

• Caseload up to 25 patients with interdisciplinary support from Physical Therapy and Social Services

Requirements:

• Active California RN license (pending license accepted, but must be licensed by start date)
• Minimum 2 years acute care Case Management/Discharge Planning experience required
• Must have hospital experience; behavioral health experience does not apply
• Previous travel healthcare experience required (first-time travelers will not be considered)
• Epic charting system experience required
• Experience with patient assessment, care coordination, discharge planning, and utilization review
• Demonstrated ability to organize and participate in family meetings and treatment plan development
• Knowledge of HMOs, IPAs, adoption/foster care, substance abuse treatment options, patients experiencing homelessness, mental health crisis intervention, advance directives, POLST, and protective services processes
• Competent with arranging durable medical equipment (DME), skilled nursing facility (SNF) placements, and ambulance transport independently
• Minimum of 2 supervisory references covering 1 year of employment within the last three years in a comparable specialty/setting
