Location: Albuquerque, New Mexico
Compensation: Details provided to qualified applicants
Schedule: 40 hours per week, self-scheduling, 5 patient visits per day, mileage reimbursed at federal IRS standard rate for 35-mile territory

Highlights:

• Provide independent home health care to adult and geriatric patients with a 1:5 patient ratio
• Work with interdisciplinary support including Physical Therapy, Interpretation Services, and Social Services
• Comprehensive 72-hour orientation program with provided MOAB certification training during first week
• Electronic charting via Cerner system in a supportive teaching hospital environment
• Mileage reimbursement for patient visits within assigned 35-mile territory
• No weekend requirement or on-call coverage needed

Requirements:

• Active New Mexico RN license or ability to obtain (pending license accepted)
• Minimum 6 months recent RN experience plus 1 year Home Health Care experience
• Must be able to function independently in home health settings
• BCLS certification required (RQI certifications not accepted)
• MOAB certification required (4-hour de-escalation course provided if not already certified)
• Must reside more than 75 miles from facility
• Valid non-local driver's license and personal transportation required
• COVID vaccination accepted with medical/religious exemptions; flu vaccine required with exemptions accepted
• Experience with adult home health assessments, wound care, catheter management, IV care, and clinical documentation
• Willingness to complete fingerprinting for CYFD upon arrival
