Job Opportunity – Registered Nurse (RN) Home Health – Georgia

Start Date: April 20, 2026
Location: Marietta, Georgia
Compensation: Details provided to qualified applicants
Schedule: Home health visits with flexible scheduling

Highlights:

Provide comprehensive patient evaluations including initial, resumption, and recertification assessments in home settings
Coordinate multidisciplinary care teams including therapists, social workers, aides, and physicians
Develop and manage individualized care plans throughout patient episodes
Deliver age-appropriate interventions addressing physical, emotional, developmental, psychosocial, and educational needs
Collaborate directly with patients, families, and healthcare team members to ensure optimal outcomes
Incorporate therapeutic, preventative, and rehabilitative nursing actions into patient care

Requirements:

Current Registered Nurse (RN) license in Georgia
Home health nursing experience preferred
Strong assessment and care coordination skills
Excellent communication and interpersonal abilities
Ability to work independently in patient home environments
Valid driver's license and reliable transportation
