Location: Santa Rosa, California

Schedule: Full-time travel assignment; weekend requirement of 2 weekends every 6 weeks; no call required

Requirements:

• Active California RN license or compact state eligibility
• Minimum 2 years of case management experience required
• Current BLS certification
• Epic charting system experience required
• Must have current work history within the last 2 years in case management or related specialty
• Experience with HMOs, IPAs, and discharge planning processes
• Competency in arranging DME, skilled nursing placement, and ambulance transport independently
• Community hospital, LTAC, or Trauma Level I/II experience preferred
• Previous travel experience strongly preferred
• First-time travelers considered with clinical approval
