Job Opportunity – Home Health RN Case Manager – Hawaii

Start Date: May 18, 2026
Location: Kaneohe, Hawaii
Compensation: $88/hour; details provided to qualified applicants
Schedule: Full-time with rotating weekends (2–3 per month); on-call 6 shifts per month as needed, including weekends

Highlights:

• Serve as a vital liaison between patients, families, physicians, and multidisciplinary care teams to optimize patient outcomes
• Lead coordination of patient care, monitor appropriateness and timeliness of services, and ensure interdisciplinary care plans align with clinical needs
• Focus on complex patient populations including frequent emergency department utilizers, chronic pain, substance abuse, and homeless patients
• Utilize Homecare Home Base and OASIS systems for documentation and regulatory compliance
• Coverage area includes Leeward Oahu communities; mileage reimbursed at $0.43 per mile
• Travel assignment with opportunity to work in a Medicare-certified home health setting

Requirements:

• Active, unencumbered Hawaii RN license (must be obtained prior to start date; allow 15–20 business days for processing)
• Minimum 2+ years of experience in a Medicare-certified home health agency (required)
• Previous travel healthcare experience (required)
• Proficiency with Homecare Home Base and OASIS documentation (required)
• Active, clear driver's license and current auto insurance (required)
• Valid vehicle for travel to patient homes across coverage area
• COVID-19 and Flu vaccinations (required; declinations not accepted)
• Bachelor's degree in Nursing (preferred)
• Case management experience (preferred)
