Job Opportunity – Hospital Case Manager – Southern California

Start Date: June 1, 2026
Location: Mission Hills, California
Compensation: Details provided to qualified applicants
Schedule: Full-time; 1:20 patient ratio; weekend rotation required to meet department needs; flexibility to commit to 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)

Requirements:

• Active California RN license (pending licenses accepted but must be active by start date)
• Minimum 2 years of acute care hospital case management experience (behavioral health only does not qualify)
• Demonstrated expertise in care coordination and discharge planning in a hospital setting
• Proficiency with Epic EMR and Microsoft applications required
• Experience utilizing InterQual criteria and understanding of DRG, CMS guidelines, and plan of care development
• Competency in independently arranging DME, SNF placements, and ambulance transport
• Knowledge of HMO/IPA processes, advance directives, POLST, and protective services
• Ability to assess complex patient needs including mental health, substance abuse treatment options, and homelessness resources
• Strong organizational skills with ability to facilitate family meetings and participate in treatment planning
• First-time travelers accepted pending clinical screening
• Up to 7 days of requested time off available
