Schedule: Monday through Friday, 8:00 AM – 5:00 PM

Highlights:

• Full-time hospice case management position with consistent weekday schedule and no weekend or on-call requirements
• Provide holistic, family-centered care focused on comfort, dignity, and quality of life for patients and families
• Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and volunteers
• Utilize electronic medical records system (HCHB) to maintain accurate, compliant documentation
• Deliver comprehensive pain and symptom management in accordance with national hospice standards
• Coordinate all patient and family services while supporting unique spiritual and cultural needs

Requirements:

• Active Registered Nurse (RN) license in the state of Texas
• Previous hospice nursing experience required
• Experience with HCHB (HomeCare HomeBase) electronic medical records system required
• Strong assessment, communication, and patient education skills
• Ability to work collaboratively within an interdisciplinary care team
