Description
Start Date: July 6, 2026
Location: Fort Worth, Texas
Compensation: Details provided to qualified applicants
Schedule: Thursday through Monday, 8:00 AM – 5:00 PM
Highlights:
- Case management role focused on holistic, family-centered hospice care
- Collaborate with an interdisciplinary team to develop and revise individualized care plans
- Electronic medical record documentation using HCHB system
- Support patients and families through end-of-life care with dignity and cultural sensitivity
- Participate in team meetings to coordinate comprehensive patient and family services
- Provide pain and symptom management aligned with palliative care best practices
Requirements:
- Active RN license in the state of Texas
- Prior hospice nursing experience required
- Experience with HCHB (Homecare Homebase) electronic medical record system required
- Strong assessment, documentation, and patient education skills
- Ability to work collaboratively within an interdisciplinary care team
