Location: Apple Valley, California

Schedule: Days, weekday ratio 1:25, weekend ratio 1:45, every other weekend rotation required

Highlights:

• Acute care case management and discharge planning role with caseloads up to 25 patients during the week
• Work with diverse patient populations including neonates, adolescents, adults, and geriatrics
• Collaborate with interdisciplinary team including physical therapy and social services
• Epic charting system in use; business attire or scrubs acceptable
• 40-hour orientation provided; up to 7 days of time off available
• Must commit to working 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)

Requirements:

• Active California RN license required (pending license accepted if active by start date)
• Minimum 2 years of acute care hospital case management or discharge planning experience (behavioral health experience does not apply)
• Previous travel nursing experience required; first-time travelers cannot be considered
• Epic charting system experience required
• Experience with assessing patients, care coordination, discharge planning, and utilization review
• Competency in arranging DME, SNF placement, and ambulance transport independently
• Knowledge of HMOs, IPAs, adoption/foster care, substance abuse treatment, homelessness resources, mental health crisis management, advance directives, POLST, and protective services processes
• Ability to organize and participate in family meetings and treatment plan development
• 2 professional references covering 1 year within the last 3 years
