Registered Nurse Qualifications

Active California RN license required
Minimum 1 year of clinical nursing experience (home health or hospice nursing experience preferred)
Current BLS certification (American Heart Association preferred)
Excellent communication and organizational skills
Ability to work independently and collaboratively within a multidisciplinary healthcare team

Home Health RN Responsibilities

Deliver high-quality nursing care to patients in their homes throughout San Jose and the Bay Area
Perform comprehensive patient assessments and develop individualized care plans
Administer prescribed medications and treatments as part of homecare services
Educate patients and families on disease management, self-care, and home health best practices
