Are you an experienced Critical Care or Acute Care RN looking for a meaningful role away from the hospital setting - one that allows you to use your clinical expertise while providing compassionate care to patients and families in their homes?

We’re seeking a dedicated Emergency Response Nurse to join our after-hours hospice and home care team. You’ll work autonomously while collaborating closely with an interdisciplinary team to ensure each patient’s comfort, dignity, and care continuity.

What You’ll Do:

• Perform thorough nursing assessments and follow individualized care plans

• Provide palliative and end-of-life nursing interventions in the home setting

• Communicate and collaborate effectively with physicians and the care team

• Offer guidance and emotional support to patients and their families

• Document clearly and provide timely handoff communication

What We’re Looking For:

• RN, licensed in Pennsylvania

• BSN required

• BLS certification required

• Valid driver’s license and car insurance

• Minimum 2 years of critical care, hospital experience
