- Job Title: Travel Hospital Pharmacist / Clinical Pharmacist

- Assignment Type: Travel Pharmacist (temporary contract)
- Duration: 13 weeks
- Shift: 8-hour day shifts
- Hours per Week: 40

Qualifications & Requirements
- Minimum 2 years recent hospital pharmacy experience (required)
- Current State Pharmacist license in South Carolina (or eligibility)
- Successful completion of educational requirements for state licensure
- EPIC experience required (electronic health record experience)
- BLS certification if required by state or facility
- Strong clinical judgment, attention to detail, and teamwork skills
Primary Responsibilities
- Manage pharmacy operations to maintain adequate drug supply and timely procurement
- Prepare, sterilize, and label parenteral medications and IV solutions following hospital protocols
- Accurately fill and dispense medications, including stat orders and unit dose systems
- Ensure strict control, documentation, and compliance for controlled substances
- Conduct monthly inspections of patient care units and pharmacy areas for safety and compliance
- Perform medication reconciliation and prepare discharge medication lists
- Provide discharge counseling and medication education to patients, families, and caregivers
- Monitor medication therapy for contraindications, interactions, dosing appropriateness, and therapeutic outcomes
- Support antimicrobial stewardship initiatives and pharmacokinetic/anticoagulation dosing management
- Participate in quality assurance, drug utilization review, and Pharmacy & Therapeutics Committee activities
- Maintain pharmacy records and ensure compliance with federal and state pharmacy regulations
- Promote a culture of safety, continuous quality improvement, and patient-centered clinical pharmacy services
