Job Details

• Location: Nashville, TN (37207) – Music City; serving Davidson & Williamson Counties (Nashville nursing jobs, Tennessee LPN jobs)
• Employment Type: Travel Contract (Travel LPN, Home Health LPN)
• Duration: 13 weeks (April 12, 2026 – July 11, 2026)
• Hours per Week: 40 (Guaranteed minimum: 32 hours)
• Shift: Monday–Friday, 8:00 AM–4:30 PM (8-hour shifts)
• On-Call/Weekend: 1 week (Monday–Sunday) every 6 weeks
• Coverage Area: Davidson & Williamson Counties (home health visits across Nashville metro)
• Dress Code: Clean scrubs
• Travel: Mileage reimbursed between patient visits

Job Requirements

• Minimum 1 year LPN experience, home health or long-term care experience preferred (Home Health LPN)
• Current LPN licensure in the state of Tennessee (Tennessee LPN license)
• CPR Certification (BLS preferred)
• Associate's Degree in Nursing
• Valid driver’s license, vehicle insurance, and dependable vehicle or reliable public transportation
• Experience with HCHB EMR (Homecare Homebase) highly desirable
• Compassionate, team-oriented attitude and strong communication skills for patient and family-centered care

Responsibilities

• Provide direct patient care during home health visits and in long-term care settings in accordance with the plan of care and state nursing laws
• Coordinate patient care with the supervising RN and interdisciplinary healthcare team
• Assess, observe, and report changes in patient condition to reduce hospitalizations and improve outcomes
• Communicate effectively with physicians, patients, families, and agency staff
• Document patient visits thoroughly in the EMR and maintain accurate medication lists
• Participate in performance improvement initiatives and case conferences
• Attend trainings and maintain clinical competencies for home health and long-term care nursing
• Maintain professionalism and schedule flexibility, including weekend rotation and holiday coverage as needed
• Adhere to agency Episode Management process and perform other duties as assigned
