Job Requirements

• Active RN license in West Virginia or compact RN license accepted
• Minimum 1 year of nursing experience; LTC, wound care, or SNF experience preferred
• Strong clinical skills in wound care management and assessment (wound care RN experience)
• Ability to work evening shifts and adapt to a fast-paced long-term care environment
• Excellent communication, documentation, and interdisciplinary teamwork skills
• Wound care certification (WCC) or WOCN credential preferred; BLS (AHA) preferred

Responsibilities

• Provide comprehensive wound care assessment, treatment, and documentation for residents in the LTC/SNF setting
• Develop and implement individualized care plans focused on wound healing and prevention
• Monitor and document patient progress, outcomes, and wound care interventions
• Collaborate with interdisciplinary team members (RNs, LPNs, CNAs, physicians, therapists) to ensure optimal resident care
• Educate residents and families on wound care techniques, prevention strategies, and post-procedure care
• Maintain compliance with facility policies, infection control standards, and regulatory requirements
• Serve as a wound care resource within the long-term care team and support quality improvement initiatives
