• Proven medication administration and resident assessment skills
• Strong communication, teamwork, and documentation skills (EHR/EMR experience preferred)
• Ability to adapt to a fast-paced environment and provide compassionate, resident-focused care

Responsibilities

• Provide high-quality nursing care to residents in a long term care/skilled nursing facility as an LPN
• Administer medications and treatments accurately and safely
• Monitor, assess, and document residents’ health status, changes, and progress in the medical record
• Collaborate with RNs, CNAs, therapists, and interdisciplinary teams to develop and implement individualized care plans
• Offer education and support to residents and their families regarding care plans and health needs
• Maintain infection control, safety, and a clean care environment in compliance with LTC regulations
