Setting: Long-Term Care (LTC) — LPN Long-Term Care
Contract Duration: 7 weeks (May 1, 2026 – June 20, 2026)
Shift: 8 Hour Evenings (3:00 PM – 11:30 PM) — Evening shift LPN
Guaranteed Hours: 40 hours per week
Schedule: Every Saturday, Sunday, and Monday; additional hours available for pickup on 3-11 shift

Job Requirements

Active LPN license in the state of New Hampshire (required)
Minimum 1 year of LPN experience in long-term care, skilled nursing, or similar LTC setting
Ability to work evening shifts and weekends as scheduled (3:00 PM – 11:30 PM)
Strong communication, teamwork, and resident-centered skills
Commitment to compassionate resident care and adherence to facility policies and state regulations

Responsibilities

Provide skilled nursing care to residents in a long-term care facility (LTC nursing duties)
Administer medications and treatments as prescribed with accurate documentation
Monitor and document resident health, changes in condition, and progress
Collaborate with the interdisciplinary care team (RNs, CNAs, therapists, social work) to ensure quality care
Support residents with activities of daily living (ADLs) and promote comfort and well-being
Maintain compliance with facility policies, state regulations, and LTC standards
