Job Details – Home Health RN / Travel Nurse

• Weekly Estimated Pay: $2,550 - $2,680
• Assignment Duration: 13 weeks (May 11, 2026 – August 9, 2026)
• Schedule: Monday–Friday, 8:00 AM–5:00 PM; weekend rotation as needed
• Guaranteed Hours: 40 hours per week
• Shift Duration: 8-hour shifts
• Territory / Service Area: Seal Beach, Huntington Beach, Westminster, Garden Grove, Fountain Valley, Santa Ana, Costa Mesa, Newport Beach, Corona Del Mar and other Orange County cities
• Productivity: 30 points per week; documentation due by 10 AM next day
• Electronic Medical Record (EMR): HCHB (Homecare Homebase)

Required Qualifications – Homecare RN / Home Health Nurse

• Active Registered Nurse (RN) license in California
• Minimum 1 year of home health experience with a Medicare-certified facility or home health agency
• Familiarity with HCHB EMR system (Homecare Homebase)
• Proven ability to work independently across a diverse patient territory
• Strong organizational, time-management, and documentation skills
• Valid driver’s license, reliable transportation, and willingness to travel within Orange County

Key Responsibilities – Home Health RN

• Provide skilled nursing care in patient homes across Orange County as a home health nurse and travel RN
• Perform comprehensive assessments, skilled visits, wound care, medication management, and skilled procedures per plan of care
• Complete timely and accurate documentation in HCHB by 10 AM the following day
• Coordinate care with the interdisciplinary home health team, case managers, and physicians
• Educate patients and families on disease management, medication adherence, and preventive care
• Ensure compliance with Medicare guidelines, agency policies, and quality standards
• Participate in weekend rotations and on-call duties as needed
