Travel LPN – Inpatient Rehabilitation & Long-Term Care in Apex, NC (27502). 13-week night shift contract for experienced LPNs supporting adult and geriatric patients recovering from stroke, neurological conditions, spinal cord injuries, amputation, trauma, and orthopedic injuries.

Job Details & Clinical Environment

• EHR: Cerner (documentation and medication administration)
• Care Focus: Adult & geriatric rehab, mobility assistance, post-acute care, wound care support

Required Qualifications

• Active LPN license in North Carolina (or compact/eligible as required)
• BLS certification (AHA preferred)
• Minimum 1 year clinical experience in adult/geriatric care
• Preferred experience: inpatient rehabilitation, skilled nursing facility (SNF), long-term care, or hospital med-surg/rehab
• Reliable, flexible, and team-oriented with strong communication skills

Key Responsibilities (LPN / Rehabilitation Nurse)

• Provide compassionate, high-quality nursing care to adult and geriatric patients in an inpatient rehab/long-term care setting
• Administer medications and treatments as ordered and monitor patient response
• Document assessments, interventions, and progress accurately in Cerner EHR
• Assist with mobility and rehabilitation using Hoyer lifts and other specialized equipment
• Collaborate with interdisciplinary team (RNs, PT/OT, case managers, physicians) to implement individualized care plans
• Support a safe environment for patients and families and escalate changes in condition per facility protocol
