- Weekly Estimated Pay: $1910 - $2010
- Location: Gunnison, CO 81230 — Colorado nursing jobs | Gunnison, CO home health
- Assignment Duration: 13 weeks (April 22, 2026 – July 22, 2026)
- Hours per Week: 36 guaranteed
- Shift: Day shift with call requirements (1-2 nights per week and 1-2 weekends per month)
- Shift Duration: 12-hour rotating shifts
- Employment Type: Travel / Travel Nurse / Travel RN
Job Requirements
- Active RN license (Nursing certification required)
- Minimum 1 year of experience in Home Health or Hospice nursing (Home Health RN, Hospice RN, or Palliative Care experience preferred)
- Colorado Adult Protective Services (CAPS) check required
- Reliable transportation required (rural Gunnison has limited public transit)
- Ability to participate in on-call rotation
Responsibilities
- Provide skilled patient care, case management, and clinical nursing for home health, hospice, and palliative care patients
- Conduct initial evaluation visits and perform ongoing re-evaluations of patient nursing needs
- Develop, initiate, and revise individualized plans of care
- Deliver services requiring substantial and specialized nursing skill in the home setting
- Initiate preventive and rehabilitative nursing procedures and educate patients and caregivers
- Prepare thorough clinical documentation and progress notes
- Coordinate services and communicate changes in patient condition with physicians and interdisciplinary team members
- Counsel patients and families on nursing care, symptom management, and end-of-life support
- Participate in in-service education and quality improvement for rural healthcare delivery
