Qualifications / Job Requirements

• Active Occupational Therapist (OT) license in Pennsylvania (PA OT license)
• Minimum 1 year of OT experience; home health OT experience strongly preferred
• Ability to travel for in-home visits within assigned Montgomery County zip codes
• Strong communication, clinical documentation, and organizational skills (EMR experience preferred)
• Commitment to compassionate, patient-centered care and evidence-based occupational therapy

Key Responsibilities

• Provide skilled occupational therapy services in both hospital and home health settings
• Perform comprehensive evaluations and develop individualized treatment plans focused on ADLs, functional mobility, and adaptive equipment
• Implement therapeutic interventions to improve independence, safety, and quality of life
• Collaborate with interdisciplinary teams (nursing, PT, social work, case managers) to coordinate care and discharge planning
• Maintain accurate clinical documentation and progress notes in accordance with regulatory and payer requirements
• Educate patients and caregivers on home programs, adaptive strategies, and community resources
