• Assignment Type: Travel / 13-week contract (travel nursing)
• Setting: Home Health / In-Home Nursing
• Start Date: May 3, 2026
• End Date: July 31, 2026
• Shift: 8-hour day shifts
• Hours per Week: 40 (guaranteed 40 hours per week)
• On Call / Weekend: Minimal — 1 day a week and 1 weekend a month
• Dress Code: Royal Blue or Black Scrubs
• Weekly Estimated Pay: $2,120 – $2,230
• Travel Reimbursement: Mileage reimbursed between patient visits

Required Qualifications

• Active, unencumbered Registered Nurse (RN) license (ability to obtain state-specific licensure if required)
• Associate Degree in Nursing (ADN) or higher
• Minimum 3 years recent Home Health RN experience (in-home skilled nursing / home care nursing jobs)
• Minimum 3 years experience with OASIS documentation and accurate clinical charting
• Strong clinical assessment, critical thinking, and individualized care planning skills
• Proven skills in IV therapy, wound care, medication administration, and patient education
• Excellent communication and patient/family teaching abilities for chronic disease management
• Valid driver's license and reliable transportation for patient visits

Primary Responsibilities

• Provide skilled nursing care in patients' homes, ensuring safety, comfort, and high-quality outcomes as a Home Health RN
• Perform comprehensive assessments and develop individualized home health care plans and goal-directed interventions
• Complete accurate and timely OASIS documentation, clinical charting, and compliance documentation
• Administer medications, treatments, IV care, and advanced wound care per provider orders
• Educate patients and caregivers on disease management, medication administration, and self-care techniques to improve outcomes
• Coordinate care with interdisciplinary team members, community resources, and primary care providers
• Participate in scheduled on-call rotation and provide weekend coverage as required
• Maintain detailed, timely, and compliant patient records to meet home health agency and CMS standards
