Job Details

Weekly Estimated Pay: $2,550 – $2,680 (competitive pay for Home Health RN jobs)
Assignment Duration: 13 weeks (5/4/2026 – 8/2/2026)
Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Guaranteed Hours: 40 hours per week
Shift: 8-hour days
On-call/Weekend Requirements: None
Territory / Service Area: Pomona, Fontana, Ontario, Rialto, Jurupa Valley, Eastvale, Chino (San Bernardino County)
Productivity: 30 points per week; documentation to be completed by 10 AM the next day
EMR: HCHB (Homecare Homebase) experience required

Required Qualifications

Active Registered Nurse (RN) license in California
Minimum 1 year of home health nursing experience with a Medicare-certified facility
Proficiency with HCHB EMR system (Homecare Homebase)
Strong organizational, time management, and documentation skills
Ability to travel within assigned territory (reliable transportation)

Key Responsibilities

Provide skilled in-home nursing care (skilled nursing, medication management, wound care) to patients in their homes across the assigned territory
Assess patient needs, develop and implement individualized home health care plans
Complete accurate, timely documentation in HCHB by 10 AM the following day
Coordinate care and collaborate with interdisciplinary team members (physical therapy, social work, physicians) to ensure optimal patient outcomes
Meet productivity standards while delivering compassionate, high-quality home health services
