• Weekly Estimated Pay: $2,030 – $2,190
• Employment Type: Travel Contract (13 weeks) – Travel Physical Therapist / Home Health PT
• Setting: Home Health & Hospice (in?home physical therapy, hospice PT)
• Location: Flossmoor, IL (60422) — Chicago south suburbs; Illinois physical therapy jobs
• Guaranteed Hours: 40 hours per week
• Shift Schedule: Monday–Friday, 8:00 AM–4:30 PM
• Shift Duration: 8 hours
• On Call/Weekend Requirement: None
• Travel: Mileage reimbursed between patient visits

Job Requirements / Qualifications

• Graduate of an accredited Physical Therapy program (DPT or equivalent)
• Active, unrestricted Physical Therapist (PT) license in Illinois
• Current CPR/BLS certification (American Heart Association or equivalent)
• Minimum of 1 year of clinical experience; home health experience strongly preferred (home health PT experience)
• Valid driver’s license, reliable transportation, and proof of auto insurance
• Knowledge of home health regulations, Medicare guidelines, and OASIS documentation standards
• Ability to perform comprehensive patient assessments and develop individualized care plans
• Strong clinical decision-making, critical-thinking skills, and ability to work independently in in?home settings
• Excellent communication skills with patients, families, and interdisciplinary teams

Responsibilities / Key Duties

• Conduct comprehensive in?home evaluations of patients’ physical condition, functional limitations, and rehabilitation needs
• Develop and implement individualized, goal-oriented therapy plans in collaboration with physicians and the interdisciplinary team
• Deliver evidence?based physical therapy interventions to improve mobility, strength, balance, coordination, and pain management
• Complete accurate and timely documentation, including OASIS assessments, in accordance with Medicare and agency guidelines
• Educate patients and caregivers on home exercise programs, safety techniques, fall prevention, and use of assistive devices
• Coordinate care with nurses, physicians, occupational therapists, and other team members to ensure continuity of care
• Monitor patient progress, reassess regularly, and modify treatment plans as needed
• Ensure regulatory compliance with federal, state, and agency policies
• Identify environmental risks, recommend home modifications or equipment to promote patient safety
• Manage daily patient visits across assigned territory while maintaining productivity and documentation standards
