Licensed Practical Nurse (LPN) travel opportunity in Zachary, LA (70791) — 8-week Long Term Care (LTC) assignment; active LA LPN required; 40 hours/week; strong wound care and home health experience preferred.

Job Details

• Shift: 8-hour shifts, 8:00 AM – 4:30 PM
• Hours per Week: 40 (Guaranteed)
• Weekly Estimated Pay: $1,370 – $1,440
• On Call / Weekend Requirement: None
• Mileage Reimbursement: Mileage is reimbursed specifically for travel between patient visits; general travel or relocation is not reimbursed.

Qualifications (LPN)

• Active Licensed Practical Nurse (LPN) license in the state of Louisiana or compact/multi-state LPN license
• Minimum 1 year of LPN experience; LTC, skilled nursing, or home health experience preferred
• Associate's Degree in Nursing or higher (preferred)
• Strong wound care, patient assessment, medication administration, and documentation skills
• Familiarity with HomeCare HomeBase (HCHB) or similar home health/care software is a plus
• Professional certification required (e.g., CPR/BLS, IV or wound care certifications where applicable)

Responsibilities

• Provide high-quality patient care in a long term care setting, consistent with LPN scope of practice
• Perform comprehensive patient assessments and accurately document findings in the medical record
• Administer medications and provide wound care per provider orders and facility protocols
• Collaborate with RNs, physicians, therapists, and interdisciplinary team members on care plans
• Maintain timely, clear documentation to meet facility and regulatory standards
• Travel between patient locations as needed; mileage is reimbursed only for travel between patient visits
