Job Details

• Weekly Estimated Pay: $2,030 – $2,190 (competitive travel PT pay)
• Employment Type: Travel Contract / Travel Physical Therapist
• Duration: 13 weeks (April 6, 2026 – July 4, 2026)
• Hours: 40 hours per week (32-hour weekly guarantee)
• Shift: Monday – Friday, 8:00 AM – 4:30 PM
• Shift Duration: 8 hours
• On Call/Weekend Requirement: None
• Dress Code: Royal blue scrubs
• Mileage Reimbursement: Yes, between patient visits (travel PT mileage)
• Coverage Area: Logan County and Todd County, Russellville, Kentucky — local home health caseload

Job Requirements

• Associate’s Degree in Physical Therapy (minimum education requirement)
• Professional physical therapy certification required
• At least 1 year of experience in home health physical therapy or related home care setting
• Experience with HCHB (Homecare Homebase) software and OASIS assessments
• Ability to supervise Physical Therapist Assistants (PTA)
• Valid Physical Therapy license for Kentucky (KY PT license)

Responsibilities

• Provide patient-centered rehabilitation as a Home Health Physical Therapist in patient homes
• Perform OASIS assessments and document using HCHB software (admissions, recertifications, 30-day assessments, discharges)
• Supervise and collaborate with Physical Therapist Assistants (PTAs) to meet patient care goals
• Maintain productivity expectations (30 points per week) while delivering quality outcomes
• Travel between patient homes across Logan and Todd counties (mileage reimbursed)
