Job Details

• Weekly Estimated Pay: $2,030 – $2,190
• Employment Type: Travel Contract (Travel Physical Therapist / Travel PT)
• Duration: 14 weeks (April 27, 2026 – July 31, 2026)
• Hours: Guaranteed 40 hours per week
• Shift: Monday–Friday, 8:00 AM – 4:30 PM (8-hour shifts)
• On Call/Weekend: One weekend per month
• Coverage Area: Clark, Crawford, Floyd, Harrison, Jefferson, Scott, and Washington Counties (southern Indiana)
• Dress Code: Any color scrubs
• Mileage: Reimbursed between visits (from first patient to last patient)

Job Requirements / Qualifications

• Active Indiana Physical Therapist (PT) license with no restrictions
• Minimum 1 year of physical therapy experience; home health or in-home therapy experience preferred
• Bachelor's degree in Physical Therapy or higher (DPT acceptable)
• Basic Life Support (BLS) / CPR certification (AHA preferred)
• Experience with HCHB (Homecare Homebase) and OASIS documentation is highly desirable
• Strong documentation, time-management, and patient communication skills for home health settings

Responsibilities

• Provide high-quality in-home rehabilitative care as a Home Health Physical Therapist
• Conduct and document start of care (SOC), resumption of care (ROC), and recertification visits
• Use HCHB and OASIS for accurate clinical documentation and regulatory compliance
• Assess patient functional status, develop individualized treatment plans, and implement evidence-based interventions
• Collaborate with interdisciplinary home health teams, case managers, and caregivers to optimize patient outcomes
• Travel within assigned counties to deliver timely, patient-centered home health services
• Participate in weekend coverage as scheduled and maintain caseload and visit timeliness
