Job Details

• Weekly Estimated Pay: $2,030 – $2,190 (competitive travel PT pay)
• Location: Marion, IN 46952 – coverage includes Wabash, Huntington, Grant, Blackford, Delaware, and Madison Counties (Indiana home health jobs)
• Setting: Home Health & Hospice (Home Health Physical Therapist / Hospice PT)
• Assignment Duration: 13 weeks (April 13, 2026 – July 12, 2026)
• Schedule: Monday–Friday, 8:00am–4:30pm; every 3rd Saturday on-call/weekend rotation
• Guaranteed Hours: 36 per week
• Shift Duration: 8 hours
• Travel: Mileage reimbursed between patient visits (travel PT assignment)
• Dress Code: Navy blue scrubs

Qualifications / Job Requirements

• DPT (Doctor of Physical Therapy) or equivalent degree
• Current, unencumbered Indiana Physical Therapy license (PT license required)
• Minimum 1 year recent experience in Home Health or Hospice physical therapy (home health PT experience preferred)
• Proficiency with HomeCareHomeBase EMR
• Knowledge of Medicare home health guidelines and OASIS documentation
• Current CPR certification
• Valid driver’s license, vehicle insurance, and dependable vehicle for patient visits
• Strong communication, teamwork, and patient-centered care skills

Responsibilities – Home Health Physical Therapist

• Perform comprehensive patient evaluations and develop individualized PT plans of care in accordance with state and Medicare guidelines
• Conduct PT OASIS visits including Start of Care (SOC), Resumption of Care (ROC), Evaluations, Follow-ups, and Discharge
• Provide skilled in-home physical therapy interventions to improve mobility, function, and safety
• Supervise and collaborate with PTAs per company policy and Indiana regulations
• Complete accurate documentation within 24 hours of each visit using HomeCareHomeBase EMR
• Participate in branch meetings, case conferences, and interdisciplinary care planning as required
• Maintain schedule flexibility to support patient needs, including weekend and holiday rotations and on-call responsibilities
• Meet productivity expectations (30 points/week) while ensuring high-quality patient outcomes
