Job Details

• Weekly Estimated Pay: $2,030 - $2,190
• Location: Centralia, WA 98531 (Centralia, Washington travel PT opportunity)
• Employment Type: Travel Contract – Travel Physical Therapist / Travel PT
• Duration: 21 weeks (April 19, 2026 – September 11, 2026)
• Hours per Week: 40 (Guaranteed minimum of 32 hours)
• Shift: 8-hour day shifts
• On Call/Weekend Requirement: None
• Dress Code: Navy blue scrubs, navy blue top with khaki shorts
• Coverage Area: Centralia and surrounding areas (mileage reimbursed between patient visits)

Qualifications / Job Requirements

• Active Physical Therapist (PT) license (WA licensure or eligible to practice in Washington)
• Minimum of 1 year of home health experience as a Physical Therapist or Travel PT
• Associate's Degree in Physical Therapy or higher (Bachelors or Doctor of Physical Therapy preferred)
• Proven ability to work independently, manage a caseload, and provide home health physical therapy
• Strong communication, documentation (EMR), and organizational skills
• Home health experience and travel PT experience are highly valued

Key Responsibilities

• Provide high-quality rehabilitation and home health physical therapy to adult and/or geriatric patients in their homes
• Evaluate, plan, and implement individualized treatment programs including therapeutic exercise, gait training, balance interventions, and manual therapy as appropriate
• Perform thorough assessments, develop goals, and modify treatment plans to optimize patient outcomes
• Collaborate with interdisciplinary care teams, caregivers, and families to coordinate care and support safe discharges
• Document patient progress accurately and timely in the EMR and maintain compliant clinical records
• Travel between patient locations within Centralia and nearby areas (mileage reimbursed)
• Meet productivity expectations while delivering patient-centered care and education
