• Contract Duration: 17 weeks (5/4/2026 – 8/31/2026)
• Hours: Guaranteed 40 hours per week
• Shift: 8-hour day shifts (8:00 am – 4:30 pm)
• Coverage Area: Up to 50-mile radius (mileage reimbursed between visits)
• Dress Code: Business Casual

Job Requirements

• Doctoral degree in Physical Therapy (DPT required)
• Current CPR certification
• Minimum 1 year of experience as a Physical Therapist; prior Home Health experience preferred
• Familiarity with HCHB EMR preferred (experience with EMR documentation a plus)
• Ability to work independently as a home health physical therapist and manage a caseload within a 50-mile radius

Responsibilities

• Perform Start of Care (SOC) evaluations, recertifications (ROC), and routine patient visits as a Home Health PT
• Develop, implement, and adjust individualized treatment plans to improve patient mobility, safety, and function
• Document patient progress, outcomes, and professional notes using EMR systems (HCHB familiarity preferred)
• Collaborate with interdisciplinary care teams (nurses, OTs, social workers) to ensure comprehensive home health care
• Travel between patient locations within the designated coverage area (mileage reimbursed)
