Job Details

• Location: Kissimmee, FL 34741 (serving Osceola & Orange County)
• Setting: Home Health / In-Home Physical Therapy
• Employment Type: Travel Contract (Travel Physical Therapist)
• Start Date: April 15, 2026
• End Date: July 13, 2026
• Duration: 13 weeks
• Shift: Monday – Friday, 8:00 AM – 5:30 PM (8-hour shifts)
• Guaranteed Hours: 40 hours per week
• On-Call/Weekend: Orthopedic weekend rotation required
• Weekly Estimated Pay: $2,030 – $2,190
• Travel Reimbursement: Mileage reimbursed between patient visits

Qualifications / Job Requirements

• Active Physical Therapist (PT) license in the state of Florida (licensed PT in Florida)
• Master’s degree in Physical Therapy or higher (DPT preferred)
• Minimum 1 year of Home Health Physical Therapy experience or equivalent
• Proficiency with HCHB (Homecare Homebase) and OASIS documentation
• Strong clinical assessment, treatment planning, and patient education skills
• Ability to manage an independent home health caseload and reliable transportation for travel between patient homes

Primary Responsibilities

• Provide high-quality in-home physical therapy care to patients across Osceola and Orange County
• Perform comprehensive patient evaluations, develop individualized care plans, and implement evidence-based interventions
• Document patient progress accurately using HCHB and complete OASIS as required
• Coordinate care and communicate with interdisciplinary teams, case managers, and family members to optimize outcomes
• Participate in orthopedic weekend rotation and manage caseload priorities as assigned
• Educate patients and caregivers on home exercise programs, safety, fall prevention, and functional mobility
