• Mileage Reimbursement: Yes, between patient visits (from first patient to last patient)

Qualifications / Job Requirements

• Bachelor's Degree in Physical Therapy (minimum requirement); Doctor of Physical Therapy (DPT) preferred
• Active and valid Physical Therapist license (PT) in Pennsylvania
• Minimum 1 year of physical therapy experience; home health experience preferred
• Experience with Homecare Homebase (HCHB) and OASIS documentation is a plus
• Strong clinical assessment and manual therapy skills in a home health/rehab setting
• Valid driver's license, reliable vehicle, and ability to travel between patient homes
• Excellent communication skills and ability to collaborate with interdisciplinary teams

Responsibilities – Home Health Physical Therapist

• Provide high-quality, patient-centered home physical therapy services to adults in Mifflin and Juniata Counties (Lewistown, PA)
• Conduct comprehensive evaluations, develop individualized treatment plans, and perform progress assessments
• Deliver evidence-based therapeutic interventions focused on mobility, functional independence, pain management, and fall prevention
• Document patient care accurately and timely using required systems (OASIS/HCHB when applicable)
• Coordinate care with nurses, occupational therapists, speech therapists, and primary care providers to optimize outcomes
• Travel between patient homes within the assigned service area; mileage reimbursement provided
• Educate patients and caregivers on home exercise programs and safe functional strategies
