Job Details

• Assignment Type: Travel, On-site (Home Health / Rehab focus)
• Duration: 15 weeks (04/20/2026–07/31/2026)
• Shift: 8-hour shifts; specific schedule to be discussed
• Hours per Week: 40 (32 guaranteed)
• Weekend / On-call: Rotation every 8 weekends (to be discussed)
• Coverage Area: Cambria, Northern Cambria, Indiana County (mileage reimbursed)
• Dress Code: Royal blue/black scrubs or khakis and polo

Required Qualifications

• Master’s degree in Physical Therapy (PT) or equivalent
• Current, active Physical Therapist license in Pennsylvania or eligibility to obtain PA licensure
• Professional certifications as required for Physical Therapy practice
• Minimum 1 year of PT clinical experience preferred (home health or outpatient rehab experience helpful)
• Valid driver’s license and reliable transportation for home health visits (mileage reimbursed)
• Strong communication, documentation, and organizational skills

Key Responsibilities

• Provide evidence-based physical therapy services to home health patients to improve mobility, function, and quality of life
• Assess patient needs and develop individualized treatment plans focused on functional goals and home safety
• Deliver hands-on therapy, therapeutic exercise, gait training, balance and fall prevention interventions
• Collaborate with interdisciplinary care teams (nurses, OT, SLP, case managers) to ensure comprehensive rehab care
• Document patient evaluations, progress notes, and discharge summaries in accordance with regulatory and payer requirements
• Educate patients and caregivers on exercises, assistive device use, and home modification strategies
• Participate in the scheduled weekend/on-call rotation as required

Preferred Skills & Certifications

• Home health physical therapy experience (preferred)
• Experience with pediatric, adult, or geriatric rehab populations a plus
• CPR/BLS certification
• Familiarity with electronic medical records (EMR) and mobile documentation
